Complete this resource with the person receiving care whenever possible. Use plain, observable descriptions—for example, the assistance needed to stand from a chair—rather than labels such as “difficult” or “high needs”.

Use this worksheet at the point of care or handover, then add the next action and the person who owns it. Store or share only the minimum information needed for that purpose.

Store the current version where authorised family members and the caregiver can reach it, date every revision, and clear out superseded instructions. Keep identity documents and unrelated medical records elsewhere — a planning sheet circulates too widely for them.
